Does an unplanned event cause your blood sugar to suddenly rise? Does your blood sugar bounce up and down to the tunes of your mood/feeling? You did not cheat in food, you did not miss out on your work out, still the sugar running through your body is rocket high?
A very critical thing to understand here is, we are not different, we have to do things differently.
Am 26 year old, living with Type 1 Diabetes since 1 year and 7 months.
I would like to take you through my journey from 380mg/dl blood sugars pre-audition of Mrs. Fabb India, Mumbai to a 80mg/dl blood sugars Pre finale day in 14 days
For all those who do not know about Mrs. Fabb India, Mumbai, it is a beauty pageant contest held by Qunix Advertising, at city level: Mumbai. It had a photo selection round followed by live training for a week and then the final contest.
What did I do differently during training to avoid a hypo?
I believe in tackling challenges which are under your control first.
1. Planning my day, one day in advance:
I would get to know about the training schedule timing 1 day in advance. I would jot down a plan with tentative time slots with activities besides it. So if my training was from 1 pm to 5 pm I would ensure that my meals and exercise schedule revolved around it.
2. I did not allow myself to sit idle:
I would have my mini breaks. Come on, we all need them. But I did not allow myself to have any major break where I did not know what to do? While schedules of practice got delayed, I would look up for some news on Google, or even click some selfies along with my newly made friends. Each of the contestants being marriage would have their own set of challenges, I would sit and hear them. I would never add diabetes to the challenge list. Because I no longer believed it to be a challenge. This helped me to widen my horizon of thought towards other beautiful experiences awaiting me with arms wide open. Do you think I got any time to think about my blood sugars? No way.
3. Maintain routine discipline:
As Dr Deepak Dalal preaches about the smooth functioning of a car depends on its 4 wheels. Even if one wheel is punctured the car will not move ahead. All the four wheels were under my control. I Exercised twice a day, Took proper Insulin doses as prescribed, ate my meals and my mid-meal snacks on time and as prescribed in my diet chat and I ended my day telling myself, “As long as you are doing everything right, your sugars will be right”, this was to calm my over-enthusiastic mind from thinking about hypo sugars.
4. Kept people around me well informed:
I shared my health and medical care with the organizers and informed them about the symptoms and the action to be taken in case of an emergency. I also shared the contact details of a family member with the organizers which got me ready for unforeseen situations.
The finale:
The day has arrived. In the afternoon, post lunch I recorded a sugar reading of 80mg/dl. I took the necessary remedies to avoid a Hyper Blood sugar and I walked the ramp on the final day in a smashing 140. It was a conscious effort by me, to not get anxious like I did during the audition 14 days back. It showed on my face, smiling away I overcame this challenge of anxiety and won the Mrs. Fabb India, Mumbai (Best smile) 2019.
Do you still think Type 1 diabetes is a drawback? I think it’s an opportunity to eat right, exercise right, and feel right which you never did before.”

Silent Heart Attack in Diabetics
A 68 years old Male living with Type 2 Diabetes visited us on 28thmarch 2019 for uncontrolled diabetes. Inspite of HbA1c of 13.5%, patient had no complaints typically associatiated with Diabetes (Hyperosmolar Hyperglycemia). Has a history of Knee pain and itching at genitals since long.
As port of our clinic protocol we asked him to get a few preliminary blood tests done along with ECG and fundoscopy (to be done annually for all diabetes).
Fundoscopy report was normal and remark on ECG report showed WNL which stands for Within normal limit.
When Doctor evaluated the report he immediately asked the patient whether he has a history of heart issues as the ECG clearly indicated an OLD HEART ATTACK. Patient said he had no history or any symptom related to heart ailment. Thus this was a case of silent heart attack
The important point that we want to bring to notice is that it’s very important to get all medical related report to be accessed and interpreted by the specialist.
Since it was a Old Heart Attack nothing can be done about it but corrective and preventive step can be taken to avoid similar incident in future with correct counseling and guidance.
Precautions and Prevention Steps to avoid further complications:

To maintain waist line below 80 cms for femails and 90cms for males.
Blood pressure less than 140/80mmhg
HbA1c below 7%
Fasting blood sugar below 120mg/dl and Post Prandial blood sugar below 160mg/dl

Gaurav Malvankar is a 28 yrs old male with Type 1 Diabetes since 10 years, very enthusiastic human being, always ready to try different ways to learn and control his Diabetes by staying fit.
He went to UK for his further studies and for work and also learnt a course called DAFNE (Dose Adjustment for Normal Eating) from that time he was eating non vegetarian diet and use to adjust his insulin dose after calculating his carbohydrate content in food by himself and ended up eating more carbs and taking more insulin, but he couldn’t approach the method properly. Because of using DAFNE in wrong way, he was under false understanding that he can eat high carb, high fat food and then counter it with more insulin dose, which was wrong.He came to India to meet family and as he was suffering from numbness in feet so then he consulted Dr. Deepak Dalal. At the time of consultation few parameters were noted, and he was put on Vegan Diet. From where his real journey started, a challenging lifestyle modification for a better health approach. During his journey of lifestyle changes he was put on Continous Glucose Monitoring System (CGMS) to monitor diet, exercise and insulin along with the variability in his blood glucose levels. CGMS helped in better understanding of effect of food and diet on blood glucose level throughout the day. He approached the method and followed diet and exercise and with that he could visibly see the changes in Blood glucose levels, his insulin dosage had drastically reduced. Gradually weight and waist also reduced. So he used the four wheels of life by monitoring blood glucose levels, taking insulin in correct method, eating right food, and exercising everyday and used these four wheels to ride his life car safely.

A few months back, I woke up to a rude shock of being Type II Diabetes. Having a family history and my craving towards sweets, it was not all unexpected, but then now it was the time to face it.
Lots of advises started pouring in.; Stop this and eat that only, take medicine regularly, you have to be careful now onwards…..Etc…Etc.

I was really shocked…what are you talking Sir? I have never heard of a person getting rid of this monster just by reducing weight and maintaining diet and that too by stopping the medicine. But he insisted and was firm on his words. Shocked and surprised, I decided to take up the challenge. He showed me results of DIRECT study showing reversal of diabetes in UK.
I am on with his suggestion and keeping my fingers crossed till my next visit in the month of November 2018 and looking forward to carry the tag: “Healthy And Happy Human Being” or you can say a person without Diabetes.
We as a family are firm believers in “Natural and Drug free living”. We believe that the mind & body have the power to heal.
Considering we don’t believe in medication, to find a doctor who has an inclination towards natural living was hard. But, making sure her diabetes was in control was of utmost importance to us.
We followed the natural way by eating right, exercising daily and most importantly thinking right yet we felt the need to have a professional who could handle it from a medical point of view. 68year old Mrs Hansa Bhatt was recently bestowed with the Title ‘SWAYAMSIDHA’ by Brahman Samaj Mahila Mandal Charitable Trust at a recently conducted function at Kalidas Natyagruh- Mulund, Mumbai. She is working with AlWC and is also associated with ‘Akhil Hind Mahila Parishad and Ankur’ and had collaboration with Fevicol Company as an artist in the past.
The back drop of the story is interesting!
Being a diabetic since a few years, she had a peculiar problem. She would work hard to bring her weight down and control her blood sugars. Would swim regularly and try her best at eating healthy. But her levels never came down. On probing once she mentioned that she would get stressed when her house keeping staff used to not follow and execute her instructions completely. This daily house hold chores related stress would spike her blood sugars. Being perfectionist by nature about the work, this agony became a routine. Married to a renowned Bollywood writer and actor she also had a lot of free time in hand since her husband’s work included long travels and hectic work schedules.
During one counseling session she happened to tell Dr.Dalal about her artistic skills and her past work in art. Doctor tapped on this point and pursed Mrs.Bhatt to pursue this skill once again. Along with being creative, the positivity she had with in, helped her accept this challenge. When you do what you like, happy hormones are released in the body which helps you calm down. She soon started spending her time in artwork, taking classes for underprivileged children, keeping exhibition for charity, gifting the artifacts she would make etc.
This led to magic..! She was off INSULIN…
HOW…?
Stress and high blood sugars are old friends. As her time was invested in productivity she started let going of the stress build up she had. And along with following diet and exercise she started showing good results and her blood sugar control to a point were there was progressive decrease in her insulin doses and finally one day her insulin was stopped and she was on basic medications to maintain blood sugars. As of now she 9 kgs. down and awaiting for the routine 3 monthly Hba1c test which is due.
Importance of Insulin Injecting Site Rotation
On 2nd January 2018 at 2 a.m. in the early morning Mr.Keshav had a severe episode of hypoglycemia (Low Sugar Attack) with random blood sugar 47 mg/dl. (blood sugar less than 70 is dangerous to life). He was semi conscious but was able to follow instructions. He was quickly helped by his wife and other family members. They gave him little sugar & salt water, one banana and few biscuits. After eating he started feeling little better and went back to sleep. Next day out of fear he did not take insulin and started insulin a day after i.e on 4th January, 2019. Post 5 days of this episode he came to our clinic for checkup.
Mr.Keshav is an advocate by profession, retired since 7-8 years due to lesser productivity at work and loss of memory issue. He also has Hyperlipidemia, Hypothyroidism, Benign prostatic hyperplasia, Hypertension, and had an episode of Cerebrovascular accident-CVA in 2003 (memory issue can be related to hypothyroidism and CVA and both need proper medical attention). His two toes were amputated in 2011 and since then has slight loss of balance while walking yet manages to walk and remain active. In 2016 he suffered a severe episode of UTI (Urinary tract infection) and needed hospitalization and took 2 months to recover. After this episode hyponatremia i.e. low sodium in blood was detected and he was advised to take extra salt daily as he felt Giddy if serum sodium went low.
Mr.Keshav is a strong willed, thin frail man weighing 52 kgs and standing tall at 168 cm. He was not regular with follow up with us and visited us after 6 years for a general check up and was found to have hyperglycemia and postural pedal edema. As a mandate during physical assessment of the patient at clinic we revise patients insulin injecting technique and insulin injecting sites and Mr.Keshav was found to have lipohypertrophy at many sites due to no site rotation and injecting insulin at the same spot for years together.
During this visit his insulin doses were kept same (i.e. Injection Human Mixtard 50:50 16-0-6 units before food) and injection technique and diet was thoroughly revised after which he observed a drop in his blood sugar and suffered a hypoglycemia attack on 2nd January.
This incident is a classic example of correcting insulin injection technique leading to better sugar control. His insulin doses were further reduced from 22 units to 6 units a day and he went home as a happy man. Post this he was taught dose adjustment and he self adjusted his insulin and started reducing his insulin doses as per his blood sugar readings and blood sugars were well controlled. He was counseled by our dietician about correct eating pattern, treatment of hypoglycemia and for his pedal edema he was advised leg elevation.
Points to Ponder:
Blood sugar 47 mg/dl is considered as moderate hypoglycemia and if not corrected then blood sugars may fall even more and patient may go into coma. So regular monitoring of blood sugars is very important to prevent such attacks.
It’s very important to check blood sugar post correction of low sugar attack:
In this case patient went back to sleep without checking, giving rise to potential episode of low sugar again..
Low sugar attack treatment.:
Check your blood sugar. If it’s below 80 mg/dl, consume 15gms regular powdered sugar or glucose powder.
Recheck your blood glucose again after 15 minutes. If still below 80 mg/dl, repeat consumption of 15 gm sugar.
Once blood glucose returns to normal, eat a small snack .
Patient has a tendency for hyponatremia, hence it is important to check serum sodium and creatinine regularly and evaluate for cause of hyponatremia and treat it accordingly.
After suffering from the low sugar attack, Mr.Keshav did not have insulin the next day. It’s important to understand not to omit insulin but readjust the insulin injecting technique and the insulin dose under a doctor’s guidance.

I monitor my blood sugars regularly and I feel blessed to share that my results are just as normal. I was very fortunate and blessed in having a lot of support from my wife, Dr. Dalal as my doctor and miss Trishala as my dietician.
I am 32 years old and was diagnosed with diabetes in February 2020. The news hit me really hard, as I was not expecting to have a chronic illness of this sort or any sort for that matter at this young age. My wife took the lead and searched for doctors on Practo. Since co-vid had taken a grip on the city and govt restrictions were all over the place, we decided to take online consultation.
My wife contacted Dr. Dalal’s clinic and we got the appointment for the next day. My medication was changed and reduced to lighter doses by Dr. Dalal.
His Teammate dietitian Trishala Goswami, who is a remarkable dietician, asked me to follow a diet plan. The best part about her diet plan was I didn’t have to make many changes to what I was already eating, but clearly enhanced it in many ways.
Trishala made my plan revolve around the food I was eating and foods I liked and did not alter my diet altogether. I kept following the diet plan, exercising regularly, sleeping at the proper time, & not keeping myself awake till late at night. I lost 4 kgs & a few inches with these changes.
The basic idea was to follow a healthy lifestyle holistically, not just focusing on diet or exercise. And yes! I have started “meditation” too. I had my appointments fixed at regular intervals to monitor my sugar levels (all of this was happening online) and every time we met Dr. Dalal, he would just infuse me and my wife with so much of positivity and reassurances that we also became definite about reversing my diabetes.
My dosages started to lower down with every appointment and alas! My medication was stopped.

A few tips in exercise regime. An approach to health assuring diet. The 16-hour fasting cycle program with supporting evidence from Nobel Prize Winning scientists. And much more. Rich material with easy access. Educative communication assumes a form of supportive treatment and divinity-blessed cure. Here it is for reaffirming.

Let divinity continue showering blessings on you. That will spread Inspiration-Guided Wisdom, and hence peace, amongst those who are fortunate to embrace. Gradually, freedom from fear and apprehension should become a corollary.
